Instructions for filling out the application form for a Bulgarian visa type “D”.

So, let's start filling out the application for a Bulgarian visa (hereinafter referred to as the "application form"). Print out the questionnaire from our website  for the Embassies of Bulgaria in Russia, Kazakhstan, Belarus , for the Embassies of Bulgaria in Ukraine. 
Let's move on to the general requirements for filling out the questionnaire.
– The application form is filled out on a computer, or by hand in a clear, neat handwriting, preferably in block letters in blue or black ink. Art in filling out will be superfluous. – Write the name and surname in Latin, exactly as in your passport, and all the basic information in the application form is filled in Cyrillic. – You will need to fill in all the items of the questionnaire, except for the column that is located on the right side of the questionnaire, it is intended for service notes. – If you have any difficulties in choosing an answer, it is better to leave the item unmarked/answered and consult with an employee of the Visa Application Center when submitting documents. – In the points where you can choose several answer options, mark the correct options with a "V" tick or an "X" cross. – No more than 3 corrections are allowed in the application form.  If you make a mistake, just carefully cross out the incorrect data with a horizontal line and write the correct option on the remaining free space. – Dates are indicated in the format day, month, year – DD.MM.YYYY. For example, the date is April 29, 1986, we write as 04/29/1986. And the phone number must be indicated along with the state and city codes. – Upon completion of the application form, carefully check all the data so that they completely coincide with the information in the passport. V N I M A N I E!! You will not accept questionnaires with shifted columns and questionnaires stretched over more than 3 pages.
And now let's take a closer look at all points.
Meet the instructions for filling out the questionnaire to receive Type D visas, as an example, on the basis of "sales representative of a foreign company", "pensioner", "family member". Paragraphs 1 and 3. Fill in in accordance with the passport. You do not need to indicate the patronymic. Paragraph 2. It refers to women who changed their surname when they got married. It is necessary to indicate all your previous surnames. If you have never changed your surname, do not indicate anything. Paragraph 4. Indicate your date of birth in the "day-month-year" format. For example, 01/31/1978  Paragraph 5. Indicate the place of birth in exact accordance with the corresponding column of the passport. Paragraph 6. We also look at the passport. If it says USSR, write USSR. Paragraph 7. "Citizenship at the present time" – everything is clear here (Russia, Ukraine, Kazakhstan, etc.). "Citizenship at birth, if different"   If you were born in a country other than the USSR or Russia, write your country of birth. Paragraph 8. Indicate the gender. Paragraph 9. Tick the appropriate marital status. Paragraph 10. It is filled out only for minor children. Either a parent or a legal guardian is indicated here. Paragraph 11. It is not required, but you can specify the number of your Russian passport. Paragraph 12. Put a mark on which category your passport belongs to. Paragraph 13. Write only the passport number in accordance with the spelling in the passport. Paragraphs 14 and 15. Indicate the date of issue of the passport and the expiration date. Paragraph 16. Indicate the authority that issued the passport (as indicated in the passport) in Latin letters and do not forget about the numbers, if any.  Paragraph 17. Indicate the address of actual residence. This is the address where you currently live. This is especially true for those who are registered in another region of Russia, but rent an apartment, for example, in Moscow or the Moscow region. The consulate will not write you letters, they need to understand where you are right now. Moreover, you may not have a residence permit at all, but you live somewhere, and this is the address that the Consulate is interested in. Do not forget to write the index. Enter your email address (personal or work, it doesn't matter). Important! Phone number! It is advisable to specify an additional phone number in addition to your mobile phone. Paragraph 18. If you are a citizen of Russia, put a tick / cross in the column "NO" and move on to the next point, while foreigners mark "YES" and enter the number and validity of the document allowing you to stay in the Russian Federation. Paragraph 19.  A relevant profession.  For representatives of foreign companies, you can write the current profession "sales representative", "manager", etc., for the director – "director". For pensioners and for family members who do not have their own basis, it is possible to indicate the past profession or profession on the basis of educational documents. Paragraph 20. Address and phone number of the employer (foreign company).  For representatives, the address and phone number of the parent company are written.  For the unemployed, pensioners, housewives and schoolchildren, preschoolers – address of residence and contact phone number. Paragraph 21. Choose the necessary reason for the trip. For representatives, you can write – "other (specify): Business", "other (specify): Business trip" For pensioners – "other (specify): Wealthy pensioner", For family members – "other (specify): Family reunification". It is also possible to put the basis for obtaining a visa according to the number of the relevant article of the Law on Foreigners. Paragraphs 22 to 23.  Bulgaria. Paragraph 24.  Check the desired number of entries – "Repeatedly". Paragraph 25.  We indicate the number of days of stay – 180; Paragraph 26.   Schengen visas issued in the last 3 years.  If you have had any of the Schengen visas in the last three years, check "YES" here, even if this visa was pasted into your old passport. In the application form for a type D visa to Bulgaria, you can enter a maximum of the last two Schengens if they were issued no earlier than three years ago. If visas have been issued before, or there were no visas at all, put a "V" next to "NO". Paragraph 27.  Previous fingerprinting for the purpose of applying for a Schengen visa.  Despite the fact that Bulgaria at the time of writing our article is not a full member of the Schengen Area, It still has information from the general database. This was done to increase the level of immigration security. Answer the truth: if you have already passed fingerprinting for Schengen, mark "YES", and if you remember the exact date, write it. If you don't remember, don't write it in, and you can do it, and if you haven't given prints at all, check the "NO" box. It's simple, friends. Paragraph 28. To be completed only if you are requesting a transit visa. You need to indicate whether you have permission to enter the country of final destination. Paragraph 29. Indicate the date that will be obtained if you add 30 days to the date of submission of documents for visa D. Please note that the beginning of the validity of medical insurance, rental agreement should not be later than this date. Paragraph 30. Specify the date that will be obtained if you add 180 days to the date specified in clause 29. Paragraph 31.  For property owners "private property, owner's full name, address",  for rented real estate "address from the lease agreement, number of the lease agreement, full name of the owner and his phone number" Paragraph 32. For representatives, it is written – the name and address of the trade mission in Bulgaria, full name and phone number of the contact person in Bulgaria. Paragraph 33. Mark a few lines: "The candidate himself", "Cash", "Credit cards", "Other" – medical insurance. This can also be written for family members, despite the declaration from the main holder of the status of a declaration of maintenance ("podslon"). For children, you can indicate in the column "other" – "provided by parents". Paragraphs 34 to 35. Not filled. It is filled out only if you have a relative in the European Union.  If there is none, then leave the clause blank. If there is a relative, write the degree of kinship. Paragraph 36.  Place – the place where the documents are submitted. Put the date of signing the questionnaire. Paragraph 37.  Signature.  For minors, the signature of the person exercising the rights of a parent/guardian. The rules are as follows: If the applicant is 18 years old or older, the application form is signed with one signature of the applicant. If the application is submitted for a child between the ages of 14 and 17, the application form is signed by the child and one of the parents/guardians. From 0 years to 13 years inclusive, the questionnaire is signed by one of the parents. Putting signatures.
